On April 1st, 2008, the Commission for Motion Laboratory Accreditation, Inc. began accepting applications for laboratory accreditation from the general public. Use this website and specifically this application portal to submit your completed application, including all supporting material.

If you are a new applicant, you must register first.
Once your registration has been processed, a online forum will be created to receive your application materials. Please see the following for uploading your initial application forms to your assigned forum:

An application fee of $750 USD is required to process your application and begin the review process.
A check or money order made payable to the Commission for Motion Laboratory Accreditation, Inc. should be mailed to:
